Explanation of Requirements for School Meal Modification Requests
Dear Parent/Guardian:
Your child’s school:
- Will make meal modifications prescribed by a licensed physician to accommodate a disability. Properly completed and signed paperwork is required as noted below.
- Will make meal modifications prescribed by a medical authority due to a food allergy/intolerance or other medical condition that does not rise to the level of a disability. Properly completed and signed paperwork is required as noted below.
- Will make substitutions for fluid cow’s milk due to a food allergy/intolerance or for other reasons. Accommodations for this request include the provision of an alternative beverage (water) or lactose free milk. Other forms of non-dairy fluid milk substitutes will not be provided
The required form can be obtained Here>>Medical Authority Modified Meal Request Form or from the Building Secretary or District Nurse. Special diet accommodations will be approved based on the information supplied by the prescribing Medical Authority. No changes or adjustments may be made by the parents/school/department without written approval from that medical authority.

Students with Disabilities/Handicaps – Any person who has a physical or mental impairment which substantially limits one or more major life activity/activities. Please note that severe food allergies (e.g. life threatening peanut allergies) fall into this category.

Students with Other Special Dietary Needs – Food allergies (that are non-life threatening) and food intolerances fall into this category. As do other medical conditions that do not rise to the level of a disability/handicap.
To ensure the requested meal modifications can be made as soon as possible, return the completed Medical Authority Modified Meal Request Form to your school’s Building Secretary or District Nurse. Families will be notified as a start date for the meal modifications. Please send an alternative lunch for your student until then. Parents may provide an alternative milk substitution for Food Service staff to serve for their student(with prior arrangements)
